The Central Transcript Office is closed effective March 16 till further notice. Due to the closure of the schools/administration offices, we have altered our service to fulfil requests for student information working remotely.

Transcripts/Student Records

All students studying in Toronto have an Ontario Student Transcript (OST). Transcripts are records of student achievement and are required for post-secondary institutions, employers and for a number of other purposes. All school boards in Ontario are mandated to maintain the Ontario Student Record (OSR) folder including a copy of the Official Student Transcript (OST) for 55 years after the student is demitted. Transcripts are available to all former and current students on request and for a fee.

If you are no longer enrolled in secondary school, please complete a Request for Transcript Form and return to the Central Transcript Office by email or fax along with a VISA or MasterCard number and a copy of your government issued signed photo ID.
