Health & Safety Issue Details	The screws securing these shelves to the wall are coming out.This issue needs to be addressed asap.

Corrective Action	Principal to tag out equipment until repair is complete or remove equipment from the workplace.
Corrective Action Taken	I have replaced plugs and screws a few years ago and told the Math Dept. Head not to put books on top of shelves;. the weight of the books can not handle it at year end when school is done and they collect all the books for a new school year. The Math Dept,. was supposed to look for new shelving units that can be put up.
